The third level: what is enforcing your corporate-wide security policy? Evidently it's not culture. We can talk and talk about security policies, but if those policies aren't incorporated into a true security-oriented culture they *will* fail. Employees are the #1 way to stop information from creeping out the door. Train them, test them, embrace them, reward them. It works.
