Unfortunately flops happen in companies rush to be first to market. It's always a trade off between getting the product right vs. being first. It's really a sign of healthy competition and innovation.

Speaking of incremental fixes, how about Vista? When the product launched on January 30, 2007 there was quite a bit of fanfare but no substance.
